Description


This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice.  This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

The Medical Readiness Contracting Office – Pacific, Bldg 339 Clark Road, Ft. Shafter, HI 96858, intends to solicit, negotiate, and award Multiple Blanket Purchase Agreements for On Island Reference Laboratory Testing and Courier Services at Tripler Army Medical Center, Hawaii.

This is a request for quotation (RFQ).  This solicitation number is W81K02-25-Q-A026.  The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2024-01.  The Standard Industrial Classification Code is Q301 size standard $41.5M and the NAICS code 621511.  This procurement is 100% SB Set-Aside.

Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications –Commercial Items, with its offer.

The deviation to clause at 52.212-5, Commercial Terms and Conditions Required to Implement Statutes or Executive Orders – Commercial Items, applies to this acquisition. See W81K02-25-Q-A026 for the FAR clauses that are applicable to this acquisition.

The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable.

The Government intends to award Multiple Blanket Purchase Agreements

Contract Requirements – The successful offeror shall be required to meet the requirements of Solicitation W81K02-25-Q-A026 and all attachments.  Contractors shall provide their current price list along with sufficient technical information within their quote.  See W81K02-25-Q-A026 PWS Att 1 Price List.
